Registration of Interest for Enrollment at genesis combined school

New Enrollments 2023-24

Please be aware, enrollments at Genesis Combined School are in high demand and we are currently operating under a Capacity Management Plan. Due to this, we are not accepting any enrollment forms. You will need to complete a Registration of Interest for Enrollment to be placed on our school Enrollment Register. The school can only accept applications for enrollment from families physically living in the school zone. All applications will be assessed in accordance with the school’s priority area. Families not able to provide all the necessary documentation to the satisfaction of the school may not be considered for enrollment.

When submitting the Registration of Interest for Enrollment the following must also be included:

Birth verification.

A birth certificate is required Please note we do accept souvenir certificates that come from the hospital or photocopies of the original, we will also accept a valid passport.

Immunization Record.

A copy of your child’s complete immunization records must be brought with you on the day of registration. Physical and dental exams are required for entrance into kindergarten, BUT not at the time of registration.

Parent Photo Id.

A current valid driver’s license, passport, or NADRA issued photo identification card that includes your signature is required.

Additional Forms That May Be Required.

GUARDIANSHIP QUESTIONNAIRE In order to register a child you must be the

legal parent/guardian of the child you are registering if you are not the legal

parent you must provide the legal documentation to prove that you are this

child’s legal guardian.
